I have an existing wordpress website https://grandscience.org/ and a hosting account with SIteground https://www.siteground.com/. I will also provide the siteground username and password and the wp admin username and password. I will provide - an HTML mockup of the new page (attached) - an XLS spreadsheet showing the headings under which the countries are to be categorized o North America (incl. Central America & the Caribbean) o South America o Europe (incl. Russia & the Baltics) o Africa o Asia (incl. the Middle East) o Oceania (incl. the Pacific) 1. I need a captcha added to the form page submission to prevent spam bots 2. I need one additional “Other” option on the Institution Type pulldown menu 3. I need a single main menu item added (Institutions) that will link to a single new dynamically populated page on the website. The placement on the main menu in the header and the page layout have already been designed and are shown in the attached HTML that illustrates the new page. 4. The dynamically populated elements of the new page will contain data extracted from the form on the website https://grandscience.org/register-interest/ The data that needs to be extracted from the form submission is the data submitted in the Institution Name, Country and Institution Type fields of the existing form 5.
