Summary Investment Application Form: Automated PDF Generation, OneDrive and Monday.com Workflow Background Jaltech is an alternative investment manager based in South Africa. We are digitising our investment application forms. We have already designed and built the online HTML form which replicates our existing investment application form. We now require a developer to make the form production-ready and build the automated post-submission workflow. Scope of Work 1. Form Finalisation and Hosting Take our existing HTML form (see attached) and make it production-ready, including field validation and electronic signature capture Host the form on a Jaltech-owned subdomain (e.g. apply.jaltech.co.za); please include hosting/deployment setup in your quote 2. Automated Submission Workflow The key requirement: once an applicant completes and signs the online form, the submitted text and signature must be inserted into our existing PDF application template (see attached old version), so the output PDF looks like our current form with the applicant's data and signature populated in the correct positions. On submission, the workflow must: Generate a PDF of the completed application, with all form responses and the applicant's signature mapped to the relevant fields of Jaltech's existing application template Save the generated PDF to a designated folder in Jaltech's OneDrive Create a new item on a specified Monday.com board, with the submitted data mapped to the relevant columns We are open to your recommended approach (e.g. custom build vs Make/Zapier/Power Automate), but the solution must be reliable, secure and maintainable. 3. Future Form Updates Your quote must also include the cost of updating or replacing the form with a new form in future (possibly for a different investment product), so we understand the ongoing cost of rolling this out across our product range. Requirements for Your Proposal Please include in your quote: Fixed price (or clear pricing structure) for the initial build and workflow Cost per additional/replacement form going forward Proposed technology stack and hosting approach Timeline to delivery Examples of similar projects you have delivered (particularly involving PDF generation/population, signature capture, OneDrive and/or Monday.com integrations) Post-launch support terms
